Additional HIPAA Considerations

Progress Notes: Counselworx™ maintains, by Indiana State Law, notes of our sessions with you that are pertinent to any diagnosis, treatment and progress. Often times they contain unsubstantiated hypotheses or hunches of cause and effect for the purpose of designing effective treatments. Under the Federal Health Insurance and Portability Act (HIPAA) we have the right to withhold these documents from you if your counselor determines that reading them may cause you psychological harm. These documents may be released however as a matter of court order.
